Referral criteria

We only accept clients who have been on Amsterdam UMC’s (VUmc) and Psychologenpraktijk OOG’s waiting list the longest and have been contacted personally by Amsterdam UMC or Psychologenpraktijk OOG. This requires a referral letter from your GP. This letter must be addressed to Gender Clinic Mental Health and request specialist psychological care.

Due to limited capacity, we cannot accept referrals from GPs for psychological care. 

Referral criteria for breast augmentation

To qualify for breast augmentation, you must meet the following and other conditions:

  • You have a BMI of 35 or below.
  • You have been on hormone therapy for at least one year.
  • You have a referral letter from your GP or doctor – this also means your health insurer will reimburse the costs of the preliminary consultation with the plastic surgeon.
  • Please email us a copy of your Declaration of Care Pathway (Verklaring Behandeltraject), which must show evidence that you have been on hormone therapy for at least one year. Please have this declaration signed by your consultant and stamped by the hospital or clinic at which you are receiving treatment. You need this declaration to request funding for a breast prosthesis from the Dutch Ministry of Health, Welfare and Sport. We would be happy to help you to apply for the funding. 

We will schedule a consultation with the plastic surgeon as soon as we receive your referral letter and Declaration of Care Pathway and if your health screening raises no issues.

A breast augmentation at Gender Clinic costs €3,830. This covers the costs of the preliminary consultation with the surgeon, the surgery and two follow-up checks. If you have any questions, please get in touch with us here

Referral criteria for orchiectomy

To qualify for an orchiectomy (testicle removal), you must meet the following and other conditions:

  • You have a BMI of 35 or below.
  • You have been on anti-androgens (testosterone blockers) for at least one year. 

You may have an orchiectomy if you have been referred to us by: 

  • Amsterdam UMC, location VUmc
  • University Medical Center Groningen
  • One of our mental health care partners. Your mental health therapist will refer you to Gender Clinic. Your file will then be discussed in a multidisciplinary team meeting*. If your file is accepted, you will be invited to attend a preliminary consultation. This preliminary stage includes a health screening. We perform this screening to establish whether your health, weight or medication use prevent you from accessing care.

*If you are receiving treatment from Praktijk Buitengewoon, you must first attend a consultation with one of our psychiatrists. Your file may then be referred for discussion in a multidisciplinary team meeting.

Referral criteria for hysterectomy (surgery to remove uterus)
  • Your BMI must be 30 or lower.
  • Hormone therapy must be arranged if your ovaries will also be removed during the hysterectomy.
  • To access hormone therapy, you must have completed your fertility care pathway or have had a fertility consultation.
  • We cannot perform a hysterectomy if you also intend to have a colpectomy (removal of the vagina).

You can only have a hysterectomy if you have been referred to Gender Clinic by:

  • Amsterdam UMC, location VUmc
  • University Medical Center Groningen
  • One of our mental health care partners.

Your mental health therapist will refer you to Gender Clinic. Your file will then be discussed in a multidisciplinary team meeting*. If your file is accepted, you will be invited to attend a preliminary consultation. This preliminary stage includes a health screening. We perform this screening to establish whether your health, weight or medication use prevent you from accessing care.

*If you are receiving treatment from Praktijk Buitengewoon, you must first attend a consultation with one of our psychiatrists. Your file may then be referred for discussion in a multidisciplinary team meeting.

Referral criteria for mastectomy and hormone therapy

To access hormone therapy, you must have completed your fertility care pathway or have had a fertility consultation.

Your BMI must be 35 or lower if you wish to have a mastectomy.

If you are 18 years or older, we will assess on a case-by-case basis whether hormone therapy is desirable prior to a mastectomy. The arguments to perform a mastectomy before starting hormone therapy can only be put forward in a multidisciplinary team meeting. If you do not want to go on hormone therapy, you can choose to have a mastectomy at Gender Clinic once you are 18.

Gender Clinic offers hormone therapy to clients aged 18 and above. Please note that Gender Clinic does not provide paediatric care.

If you are 16 or 17 years old, you may only have a mastectomy once you have been on testosterone therapy for at least one year. If you are 18 years or older, we will assess on a case-by-case basis whether hormone therapy is desirable prior to a mastectomy. The arguments to perform a mastectomy before starting hormone therapy can only be put forward in a multidisciplinary team meeting. As a result, if you do not wish to access hormone therapy, you may only have a mastectomy at Gender Clinic after you turn 18.

You may only have a mastectomy or access hormone therapy if you have been referred by:

  • Amsterdam UMC, location VUmc
  • University Medical Center Groningen
  • One of our mental health care partners
